Free worksheet

2 pages. 15 minutes. The thing you keep putting off.

Most people don't stay stuck because they don't want to change. They stay stuck because they want two things at once — the change, and the comfort of how things are.

Ready for Change? is the worksheet that gets both onto paper.

What's on the two pages

Three steps, in order

Page one of the Ready for Change worksheet
Page 1, exactly as it arrives.
1 · Name it The change you keep circling. Getting back in the gym, the conversation with your dad, the side business, cutting back.
2 · Both sides, honestly Four boxes: what's good about how things are now, what staying the same is costing you, what would be better, what's hard about changing. All four are true at once. That's what stuck feels like.
3 · Two numbers How important is this, 0 to 10 — and then, why that number and not a lower one? Answer that and you've just argued for the change, in your own words.
4 · A year from now If you made the change and it stuck, what's different on an ordinary Tuesday? Where are you, what are you doing, who notices?
5 · The next 48 hours The smallest thing that points that direction. One text. One five-minute version. One appointment booked. Not the whole change.

If you keep telling yourself you'll start Monday — the workout, the business, the boundary, the change — this was built for the pattern that keeps stopping you.

Who made it

Michael Chester

Michael Chester, MA, LPC

Michael is a Licensed Professional Counselor in Mount Pleasant, South Carolina, and the founder of Everest Therapy Solutions. This worksheet is the conversation he starts with — the one that comes before any plan, because a plan you're only half sold on doesn't survive a hard week.

It won't try to talk you into anything. It asks the questions a good conversation would ask, and lets you hear your own answers.

If you want more than a worksheet

The worksheet names the gap. The course is about closing it.

Everything you write on page 2 is information about the distance between wanting something and doing it. Mindset Evolution is built for exactly that gap: what your brain does when you get close to change, and how to move anyway. Ten sessions, one a week, about an hour each — a video, a downloadable Session Guide, and one action you commit to.

You post that action in a private community where Michael and a group of people working towards goals of their own can see it — and they ask how it went. A hard week gets support, not judgment. If you go quiet, someone notices. That part is the difference between a course you watch and a course that changes something.

You don't need the course to use the worksheet. The worksheet is yours either way.

An important note. This worksheet is a personal growth tool built on research-based psychological strategies. It is not therapy and not treatment, and it isn't a substitute for professional support.

If you're in crisis or thinking about harming yourself, call or text 988 (Suicide & Crisis Lifeline) in the U.S., or contact your local emergency number. If you're looking for a counselor, Everest Therapy Solutions can help you find the right support.
